Middle School Peer Tutor Program Contract

Orientation
20 to 30 minute period working with Adapted P.E. staff concerning goals and objectives of program

Goal
To facilitate implementation of inclusion of students experiencing disabilities with the regular physical education class.

Objectives

1. participate in a 20 to 30 minute orientation

2. participate in regular physical education class 45 minutes per day, five times per week with student with disability

3. daily notation (one or two sentences) describing activities

4. sign confidentiality statement

5. sign peer tutor contract

6. complete evaluation

Confidentiality
I will keep confident any information pertaining to the disability of my peer buddy. I will not discuss his/her disability with my peers.
