Student-Led Conferences

As all of our middle schools move toward implementing student-led conferences, it is important for us to share our resources since we are all so busy. No reason to reinvent the wheel! So in the spirit of sharing, please consider submitting your team's organizational forms. In this section, you will find forms in the following categories: agendas, behavior checklists, miscellaneous, portfolio preparation, post conferences, and scheduling.
In some cases, you will find only slight variations on a theme and in other cases you will find forms unique to a particular teacher or school. In all cases, an attempt has been made to cite the source in order to recognize the hard work of the contributor. Some of these forms have been circulating for several years making it difficult to honor the original author. Please continue to refine, revise, and recreate forms as we learn from one another and improve our implementation. Send all contributions to Amy Goodman.
