Release and disclosure of directory information
Parents and students 18 or over, have the right to request that student directory information is not released to military recruiters and others. Complete this release and disclosure of directory information form and give it to a principal or school administrator. If there are no objections to the release of directory information, no action needs to be taken. Please note: This form, requesting that the student’s private information not be released, must be renewed
annually.
