During School Year hours as follows:
Activities Office Hours are Monday-Friday     7 AM – 2:40 PM
Student may sign-up before & after school, also during lunch.
Bring the following:
1)    ASD Health Examination Form signed by doctor, student & parent/guardian,must not expired during sports season.
2)    2009-10 High School Athletic/Activity Participation Form (not yet available) signed by student & parent/guardian
3)    ASAA "TAD" form (parents and student must watch video available at www.asaa.org and at the activities office.)
4)    Cash or Check payable to West High School for $TBD per sport/Family Cap $TBD, 09/10 fees are not yet determined .
5)     Student attending an Alternative/Charter School must bring a copy of their current transcript and schedule.
How do I know if I am eligible for sports?

• Athletes must be enrolled in at least 5 ASD classes to participate in sports
• 4th qtr/2nd sem. grades will be verified for: 2.0 GPA minimum & 5 classes passed; (this rule does not apply to incoming 9th graders until the end of 1st qtr.) 
• Seniors who are on track to graduate (have passed all HSGQE exams and have at least 17.5 credits) may enroll in 4 classes and still be eligible.
• Additional forms are required for any transfer/foreign exchange students.

You must complete the eligibility process with the Activities Office before you are allowed to begin practice with any team.  Sports Registration will begin two (2) weeks prior to the 1st practice and closes three (3) weeks after games begin.
*Refunds must be requested prior to the first game student is considered eligible.
