SIGNING UP FOR SPORTS

Bring the following:

  1. Health Examination Form signed by doctor, student & parent/guardian.
  2. High School Athletic/Activity Participation Form signed by student & parent/guardian. Available at www.asdk12.org/athletics and at the activities office.
  3. ASAA "TAD" Play for Keeps form (parents and student must watch video available at http://www.asaa.org/playforkeeps/index.html and at the activities office).
  4. Cash or Check payable to ASD (see payment schedule). Waivers are available. ***Refunds must be requested prior to first eligible game.
  5. Student attending an Alternative/Charter School must bring a copy of their current transcript and schedule.
  6. Concussion Impact Baseline Acknowledgement and $5.00 fee. Forms available at www.asdk12.org/athletics
These forms are available outside the Activities office.

HOW DO I KNOW IF I AM ELIGIBLE FOR SPORTS?

  1. Athletes must be enrolled in at least 5 ASD classes to participate in sports.
  2. Each Sem/Otr. grades will be verified for: 2.0 GPA minimum & 5 classes passed; (this rule does not apply to incoming 9th graders until the end of 1st qtr.)
  3. Seniors who are on track to graduate (have passed all HSGQE exams and have at least 17.5 credits) may enroll in 4 classes and still be eligible.
  4. Additional forms a required for any transfer/foreign exchange students.
You must complete the eligibility process with the Activities Office before you are allowed to begin practice with any team. Sports Registration will begin two (2) weeks prior to the 1st practice and close two (2) weeks after games begin.
