Physical Exam and Health History

Kindergartners should have a school physical within 90 days of enrollment or provide the school with a copy of an exam completed within the last year. A perfect opportunity to receive a PPD tuberculin skin test with parent present is during the school physical. Download the Physical Exam Form here.

New to district students in grades 1-12, transferring into the Anchorage School District from another school district shall receive, within 90 days, a health screening by the school nurse. Parents will be notified of any health problems identified during the health screening. The school nurse will:A nurse screens a student's vision

Each year when parents register their child(ren) for school and a current health history form should be completed. The school nurse reviews the health history form and parents will be contacted when their are questions or concerns.

Parents are encouraged to call or visit their school nurse and talk about the special needs of their child. This is especially critical for students with special medical conditions or when there has been a significant change in a child's health.
