Forms

Paycheck distribution request to change paycheck distribution location for temporary and substitute employees (PDF)

Payroll Closures

Payroll will be closed for the holidays on the following dates:

November 26-27, 2009

December 24, 2009 - January 3, 2010

Please return all checks & advices that were not distributed to Payroll by December 23, 2009. Do not locked them up or leave them in the staff mail boxes. Download the November holiday payroll memo.

W2’s will be issued soon. To make sure you receive your Form W2 timely, please make any necessary corrections to your address. Download the Change in Personnel Record form.
