Getting Ready for Kindergarten
Parents have questions about preparing their children for Kindergarten...here
are many of the questions and the answers that we give!
Where can I find out more about the physical examinations for
entering Kindergarten?
All children attending an Alaska school (including preschool)
MUST be immunized against hepatitis A and hepatitis
B. Any child who lacks proof of hepatitis A and B shots
or a valid exemption
will be excluded from school. Click

Once your child's shot record has been updated, be sure
the district gets a copy. Send or fax a copy to our
Health Services
Program and bring a copy to your school when you register
your child.
To register, you will need the following
health forms:
Form 301 -- Health History
For all students -- preschool, kindergarten, returning,
new to ASD, and transfer students
Form 318 -- Physical Examination for School Entry
For kindergarten students

You can contact the ASD STeP Center.
The STeP Center provides
information resources, including web and library resources,
for students, teachers and parents throughout the Anchorage
School
District. Professionals there can assist you in answering
your question or making an appropriate referral to another
