Directions
- Read ALL directions on the form carefully and completely.
- CAREFUL! When you come to the word STOP! if you have completed all the required information, scroll to Electronic Verification at the bottom of this form and continue.
- Everyone must fill in three sections of this form:
°Section 1
°The HQT Option you choose
°The Electronic Verification at the bottom.
- Follow this guide on the side of each section of hints, tips, and directions. Each section is explained below.

- Everyone needs to complete Section 1.
- If you are not sure which content area(s) covers the course(s) you teach, click on http://www.asdk12.org/NCLB in "Core Area/Teaching Assignment" to see the Course to Content List. Elementary teachers should click Elementary.
- If you are unsure about the type of certificate you have or your endorsements, click http://www.eed.state.ak.us/teachercertification under the "Certification Type" header. Follow the prompts to see your certificate.

| - If you began your teaching career prior to July 1, 2002, you may consider using HOUSSE. This includes years teaching in other districts.
Area 1)
Elementary: In any Grade Level
Secondary: In Content Area ONLY
Area 2)
Check your certificate at:
http://www.eed.state.ak.us/teachercertification
Area 3)
"Posted" means that it is listed on your transcript.
Elementary: Any minor relating to teaching Elementary grades.
Secondary: Only for minor in content area in which you are seeking HQT status.
Area 4)
Any graduate degree for both secondary and elementary.

| Area 5)
"beyond teacher preperation program"
- For many teachers, this will be your bachelor's degree. For others, it will be your graduate degree (whichever degree was your teacher prep program).
- There is no maximum for points beyond teacher preperation program.
If your coursework can be verified with transcripts already on file at HR, you do not need to submit transcripts. Request transcripts to be sent to the HR department at ASD only if they are not already on file.

| - No maximim points for documented Professional Develoment relating to your teaching assignment.
- Read definitions and examples carefully. Any professional development activities you list should match the rigor of these examples. Think about the intent of area 7 and, if in your best professional judgement, you participated in similiar activities, list them in the text box.
- Be careful! If you counted a conference under credits (Area 5), do not duplicate it by using the same conference here. You may only use each activity once.
- Credit for teacher in-services have very specifiec guidelines . Read carefully. In-services can count for no more than 10 of your total points in this area.
- You do not need to submit written verification, but you must indicate how the event can be verified.

- There is no maximim for service to the teaching profession and content area.
- Mentor Teacher refers to serving formally as a mentor through the district (AEA/ASD) or state mentoring program.
- Instruction at an Institution of Higher Education MUST be in content area for secondary teachers.
- You do not need to submit written verification, but you must indicate how the documented service can be verified.

| - Be careful. Your list of activities and awards needs to match the rigor and intent of these examples.

| - Carefully add all your points for HOUSSE and enter here.
- IF your total is 100 or more points, skip option 5 and go directly to "Electronic Verification."

- EVERYONE fills out electronic verification section.
- Be sure to read the section, then select "I Agree". This is equivalent to your signature for this record.
- Click Submit to send your application. Click Save as Draft if you need to gather more information and return to the form before submitting.
- If you click Save as Draft you will find the document in MLP on your "Learning Plan" page under DRAFTS. If you click Submit your name will appear on your "Learning Plan" page under Awaiting Approval. When your application has been approved your name will appear on your "Learning Plan" page under Approved. You will also find it under your MLP Portfolio.
