Dream Based Transition Alliance

DBTA helps families of young adults with disabilities (age 14 and older) to
navigate the maze of issues and challenges that occur during the transition
years. DBTA facilitates person-centered plans, provides information, holds
monthly Parents in Transition Meetings, assists with other emerging parent
