Gifted Mentorship Program
Application materials
To be considered for a mentorship, a student must meet the eligibility
requirements and turn in the following:

First & second semester mentorship applications due by Sept. 30, 2011 |

| Mandatory training session |
| These are the only training dates for the 2011-12 school year. All students have to attend a session to receive a mentorship this year. |

Form: Application for Mentorship

Form: Multidisciplinary Evaluation Report. Fill out the top of page only.

Rules of the Mentorship: read and sign

Two recommendations: at least one from a teacher who can evaluate
your skills and potential in your area of mentorship interest.

Your resume: Limit yourself to ONE page. See format and example.

Attachments: Copies (NOT ORIGINALS) of your highest test score report(s): PSAT,
PLAN, SAT, ACT. You choose which to include but MAKE SURE TO ONLY INCLUDE A COPY, NO ORIGINALS. If you have no reports available (such as not
having taken one of these yet), attach a note explaining why.

Only completed applications will be accepted, including a transcript, resume, the application packet, and test scores. Applications must be turned into the student's school with the designated person by Sept. 30, 2011 for all 2011-12 mentorships. |
