All students new to the Anchorage School District must meet the State of Alaska Immunization requirements for Diphtheria/Pertussis/Tetanus, Polio, Measles/Mumps/Rubella, Hepatitis A and Hepatitis B. Proof of immunization must be provided at the time of enrollment. New students will be screened by the school nurse for height, weight, hearing and vision, and a health history obtained. A tuberculin skin test will be placed by the school nurse within 90 days of enrollment, unless proof of testing within 6 months of enrollment, or history of a positive test in the past is submitted to the nurse.

Medication Policy
Long term Medications (medications prescribed for 2 weeks or longer) must be in the original container and kept in the Nurse's Office. A long term medication form or written consent signed and dated by the physician and parent is required for any medication prescribed longer than 2 weeks. Long Term Medication forms are available through your physician or the school nurse. A new form is required each school year.

Short Term Medications (medications prescribed by a physician for 2 weeks or less) must be delivered to the Nurse's Office in the original container. A short term medication form is available in the Nurse's Office and must be signed and dated by the parent. Only current prescriptions will be given at school.

Nonprescription Medications: The Anchorage School District will assist parents by administering approved nonprescription medications for a limited period of time not to exceed five days. The approved nonprescription medications, limited to Tylenol or cold and cough preparations not containing alcohol, must be delivered to the school in the original container with a manufacturer's label identifying the medication and dosage schedule. A nonprescription medication form or parent note must accompany the medication. This note must include the name of the student, medication name, date medication began, dosage, and time of administration.

Asthmatics : High school students who have asthma may carry an inhaler at school provided a Long Term Medication form is on file with the school nurse. If the student has a history of severe attacks, please consider keeping a backup inhaler in the Nurse's Office. Inhalers must have the student's name on the device.

All medications not picked up by the parent at the end of the school year are discarded.

Students becoming ill or injured during the school day should report to the nurse. If it is necessary to go home or to a doctor, the nurse will inform the parent/guardian and a Blue Pass will be issued. It is important that the school be kept informed of current phone numbers - home, work, cell, and emergency contacts.

If your student develops a significant health concern during the school year that may affect his/her performance at school, please notify the school nurse.
