2012-13 Registration/Enrollment information
Schedules not activated or picked up on the assigned day will be dropped from the computer at the end of the day and students will have to reschedule. Your elective choices may be endangered by your failure to activate.
All students must register prior to the beginning of school. If you need assistance determining the school your child will attend, please use our online boundary tools or call Demographics at 907-348-5190. Address verification will be required during registration. Families new to the district may find our Newcomers' Guide helpful.

Anchorage School District (registration form)
This is required to register all students for the 2012-13 school year. Returning ASD students DO NOT need to complete this form. They will receive a pre-printed version at registration.
Release and disclosure of directory information
Use to designate what information may or may not be released about your student.
Survey permission form
This form regards parent/guardian consent for students to participate in certain school surveys.
Media release form
Used to grant parent/guardian permission for use of student information, photos, images, etc. for use in media projects.
Internet agreement for students
Sets out guidelines and permission for use of the district's Internet equipment and systems.
Health Forms
These forms are posted as PDF documents and they require the free Acrobat Reader to be viewed. Most forms are formatted so you can fill them out online, print them and then be mailed or dropped off at the school or department. Medication Related Forms
Authorization for Self-Carry/Administration of Medicine at School and After-School Activities Form 902
Parent permission form allowing for a responsible, trained student to carry and/or self administer prescription labeled medication for asthma, severe allergic (anaphylactic) reaction, or diabetes.
Long Term Non-Prescription Medication Request
Parent permission form for requesting the district give non-prescription medicine under certain conditions.
Long Term Request for Administration of Prescribed Medication Form 317B
This form or a written statement signed and dated by the physician is required for medication prescribed for more than 15 days.
Short Term Prescription Medication Request Form 317C
Parent permission form for allowing school personnel to administer health care provider prescribed short-term prescription medicines for a period of time not to exceed fifteen days.
Exemption Forms
Religious Exemption
Use this form to request an exemption from immunizations based on the tenets or practices of a church or religious denomination.
Medical Exemption
Use this form to request an exemption from immunizations in situations where immunizations would be injurious to the health of the student or other household members.

Students Rights & Responsibility Handbook
This handbook contains most of what you need to know to have a
successful year at Clark. It will be your responsibility to read this book
and know its contents. If you do only those things you know are right and
treat others as you would like to be treated, you will have a wonderful
middle school experience. The administrators, teachers and staff are here
to help you meet your goals. Work hard, abide by the expectations, and
trust others when you need assistance, and success is all be guaranteed
during your stay at Clark Middle School.
