ACTIVITY TIDBITS

• The Dimond Activities Office is open from 7:00 AM-12:00 PM and from 1:00-2:45 PM Monday through Friday.
• Many Dimond sports and clubs have their own websites/pages. Please visit the Activities section of our Links page.
• ASD high school sports information can be viewed at http://www.asdk12.org/activities/.
• Please be aware that Dimond Activities Office cannot cash checks or make change for students during the school day. We do have a bill changer for 20’s, 10’s, 5’s, and 1’s; it gives change in quarters only. The changer is located in the student commons.
• Yearbooks can be purchased from Mr. Sanderson and the yearbook staff in room A113. Click here for more information.

MEETING NCAA REQUIREMENTS

• If your son/daughter is an athlete and has a goal to play in college sports, notify your student’s counselor to insure he/she is taking the required “Core Courses” and has the necessary information to meet eligibility. You can access NCAA information at www.ncaa.org. Additional links are available on the NCAA link page.

COACH ASSESSMENT

• Students and their parents/guardians are invited to complete a coach assessment at the end of a sport season and submit it to the Activities Office.

MEETING ELIGIBILITY REQUIREMENTS

• Athletes must be enrolled in at least 5 ASD classes to participate in sports.
• Grades from the previous semeser/quarter will be verified for: 2.0 GPA minimum & 5 classes passed (with the exception of incoming freshmen until the end of 1st quarter).
• Seniors who are on track to graduate (have passed all HSGQE exams and have at least 17.5 credits) may enroll in 4 classes and still be eligible.

2011-2012 ELIGIBILITY CHECKLIST

The following items must be submitted to the Activities Office for eligibility clearance. Forms are available in the Activities Office or online at http://www.asdk12.org/activities/high/participant_HS.asp. Do not submit paperwork to your coach until the Activities Office clears and signs it.

1. ASD Health Examination Form signed by doctor, student & parent/guardian
2. High School Athletic/Activity Participation Form signed by student & parent/guardian
3. ASAA "TAD" form---signed by student & parent/guardian after viewing "Play for Keeps" video (found at http://www.asaa.org/playforkeeps/index.html)
4. Cash or check payable to DHS (see the ASD High School Activities page for the fee schedule for various sports)
5. Concussion policy acknowledgement form, ImPACT baseline assessment, and $5 assessment fee (see the ASD High School Activities page for details and forms)
