Information to Know
The activities office is open daily from 7:00 am to 11:15 am & 12:45 pm to 2:45 pm. Students may submit activity participation paperwork to the activities office before school, during lunch and after school. Forms are available online (see links below) and at the activities office.

• Complete the top portion of the form.
• Do not write in the section below that states . . .”This section to be completed by the Activity Office.”
• Make sure parent/guardian and student signatures are on the form.
• Do not remove any page of the two-part form.

ASD Health Examination Form (Physical)

• If you have a current health form on file that will last the duration of the activity, please tell Ms. Deese when submitting your eligibility paperwork.

Activity Fee

• $160.00 per activity up to the $330 yearly family cap. We accept cash or check payable to “ASD.”
