    Sports -

       If you wish to sign your student up for a sport, complete the following:

            Student Health Review/Exam  - must be completed by doctor yearly

            Activity Participation Form - must be completed for each sport/activity

            Sports Fee Remittance Form - must be completed for interscholastic and attached to check/cash

Parent Language Questionnaire

If parents have limited proficiency in English, this questionnaire needs to be filled out s the District will be aware and can facilitate communication between the school and parents.
