Kindergartener / New to the District Requirements: Birth Certificate (copy acceptable Current Immunization Records Physical Examination (For Kindergarteners) Current physical and mailing address and phone numbers of students and parents Names and phone numbers of emergency contacts and individuals who may pick up the child from school Copies of legal documents if there is a custody situation Proof of Residency (utility bill, phone bill, etc.) Name and address of previous school attended Returning Student Requirements: Current Immunization Record Proof of Residency (utility bill, phone bill etc.) Up date current information (phone numbers, Emergency names and numbers,etc.)
