Students who will be absent for short or long term absences can take this form to their teachers to arrange for work on their absence. Return the form to the Front Office.

Students wishing to ride the Activity Bus should have their parents fill out this form and return to Ms. Debbie Oakes, Security

Homework Time

Students can use this activity on a drop-in basis to have extra time to work on homework after school. Return the form to Ms. May Smith in the 6th Grade Pod or turn in to the Front Offic

This form gives Parent Permission for all activities, i.e., sports, clubs. Return to Ms. DeFrees in the Front Office

Zone Exception Form

This form is used to request a student to be allowed to attend MLMS thought they live outside the Mirror Lake attendance area. Family must make their own arrangements to get their student to the school.

at the end of another great year at MLMS

END OF SCHOOL YEAR ON THURSDAY MAY 20TH. Teacher’s last day is May 21st. Lockers are to be cleaned out Monday 5/17 & 18/2010. On May 19 and 20 lockers will already be cleaned out and students have been instructed not to bring backpacks and/or any other items that would normally be stored in lockers (hats, etc.). Staff will post at building entrances to help students remember.
