Permission Slips

It is District Policy to have written permission from a parent for a children go on all Field Trips. For walking field trips to Pop Carr Park, next to the school, a Willow Crest Park Field Trip form must be signed and returned to school.

In addition, we take photographs in our class for use in writing and other activities. We also make videotapes of Show and Tell and class performances. These photos and videos are used only in the classroom and are not distributed in any way. A permission slip must be signed for your child to participate.
