Summary

Although the State noted several areas of strength in our IEP files, they also found several areas needing improvement, including some noted in previous years. The main points we just covered are:

ESER - Be sure to include:

Clearly document limited academic achievement

2

Clearly identify all eligibility points

3

Specific related educational needs and recommendations

PLAAFP - Be sure to include:

1

Present levels for all specific needs that were identified in the ESER

2

A narrative synopsis of the student's academic and functional performance in the identified areas of need

3

A narrative synopsis of the student's current assessment data and how it relates to his/her performance

4 Identify transition needs and review desired post-school outcomes when appropriate
5 State the effect of the disability on the participation of preschool-age children in age-appropriate activities

Goals and Objectives - remember:

That they are measurable

They align with state standards

They address each academic and functional area of need in the PLAAFP

Repeat Errors :

Services are supervised by a certified special education teacher or related services provider when they do not provide the services

Progress reports related to goals/objectives must be in the file

3

Document regular education interventions before referring a child to special education

4 When related services is provided through consult, describe the frequency of the consult and area(s) of instruction the consult applies to on page 8
5 Transportation must be included in the IEP if the student needs it, even if the parent chooses to transport

Every IEP must list General Education time, even if the time is zero hours

7 Follow the memo regarding out-of-distric IEPs
