Evaluation Summary and Eligibility Report

Overview

Monitoring 2006-2007 identified a variety of areas to improve in the Evaluation Summary and Eligibility Report (ESER). Not all areas in need of improvement were found in all IEP’s. However, the frequency and variety of errors reflects a need to improve ESERs districtwide.

The three areas identified in the audit as needing improvement are:

Clearly document limited academic achievement

  • All evaluations required for eligibility
  • Current academic achievement and functional performance based on classroom and other assessments that demonstrate the need for specially designed instruction (special education), and the nature and extent of the special education and related services that the child needs

Clearly identify all eligibility points

  • Document that the student meets each requirement for eligibility under the selected disability
    • For learning disabilities, document the processing deficits
  • Include the disability category and a statement of whether the child requires special education and related services

Educational Needs and Recommendations

  • Needs should be specific
  • Needs should be directly related to the areas of need that are documented in the assessments

Additionally, be sure to continue to include these items in all ESERs:

  • The date of the report
  • The name, birth date and gender of the child
  • The dates the evaluations were conducted
  • A description of environmental, cultural, or economic factors, if appropriate
  • The disability category and statement of whether the child requires special education and related services
    • document that the student meets each requirement for eligibility under the selected disability
    • the eligibility determination must be supported by and be consistent with the other information contained in the ESER
  • The signature of each team member, including a parent
  • Dissenting opinions, if any
