Repeat Errors
From Previous Monitoring

Overview

During the monitoring, some errors were noted that had been identified in previous years.

Please be certain to remember:

1

Services not provided by a certified special education teacher or related service provider are supervised by a certified special education teacher or related services provider

2

Progress reports related to goals/objectives must be in the file

3

Document regular education interventions before referring a child to special education

4 When related services is provided through consult, describe the frequency of the consult and area(s) of instruction the consult applies to on page 8

Transportation must be included in the IEP if the student needs it, even if the parent chooses to transport

  • In the latter case, include Parent declines transportation

Every IEP must list General Education time, even if the time is zero hours
