    The following documents are required:


    • IRS letter indicating exemption from federal taxes - Form 501(C)3
    • Certificate of Insurance in the amount of $1M, commercial general liability with subrogation waiver and list ASD as additional insured.
    • List of Board of Directors  (include name, address and phone)




    Written notification must be received 10 or more work days prior to an event to receive a FULL refund.

    Schedule changes must be requested by the permit holder via email or phone call and may be subject to a $10 change fee.
