     Written notification must be received 10 or more workdays prior to an event to receive a full refund.

     

    Changes

    Schedule changes must be requested by the permit holder via email and may be subject to a $10 change fee.

     

    Documentation Requirements

    • IRS 501(c)3 letter of approval indicating exemption from federal taxes.
    • List of Board of Directors (include name, address, and phone).
    • Certificate of Insurance in the amount of $1M, commercial general liability with subrogation waiver, and list ASD as additional insured.

    AR 1330.1 FACILITY RENTAL INSURANCE REQUIREMENTS

    Non-ASD groups requesting rental of ASD facilities must present a Certificate of Insurance to the Community Services Department (Rentals) in the amount of $1,000,000 in commercial general liability limits and include a subrogation waiver and an additional insured endorsement naming the Anchorage School District.
