Documentation Requirements
The following documents are required:
- IRS letter indicating exemption from federal taxes - Form 501(C)3
- Certificate of Insurance in the amount of $1M, commercial general liability with subrogation waiver and list ASD as additional insured.
- List of Board of Directors (include name, address and phone)
Cancellations
Written notification must be received 10 or more work days prior to an event to receive a FULL refund.
Changes
Schedule changes must be requested by the permit holder via email or phone call and may be subject to a $10 change fee.School Board Policy
