The Early Intervention Services Center (EISC) serves as the central hub for supporting families of preschool-aged children. It provides key services including:
- Child Find and Child Check: Screening and assessment to determine eligibility for special education and related services.
- Transition Support: Assisting families as their children transition from infant learning programs to school-based services.
- Out-of-District IEP Assistance: Helping families navigate the process for out-of-district IEPs for their preschool children.
Parents seeking an assessment for their child should be directed to the EISC. Additionally, the EISC staff are a valuable resource for both school teams and families, offering guidance and support throughout the process.
Developmental Screenings
EISC offers free developmental screenings for preschool aged children (3-5). If you have concerns about your child's development, please complete these steps to initiate a referral to EISC.
If your child is younger than three, please contact Infant Learning to initiate a screening. If your child lives in Anchorage, contact Programs for Infants and Children. If your child lives in Eagle River/Chugiak or on JBER, please contact FOCUS.
Step One:
Complete and sign documents via PandaDoc or download to print:
- Parent Questionnaire
- Health History Questionnaire
- Releases of Information (optional)
Step Two:
Complete both the Ages and Stages Questionnaire (ASQ-3) and the Ages and Stages Questionnaire - Social Emotional (ASQ-SE) screeners at ASQ Online. Select the option “I am completing both the ASQ-3 and ASQ:SE-2 questionnaires.”
Step Three:
Provide the following documents to EISC via PandaDoc, email, fax, or mail:
- Copy of your child’s birth certificate
- Immunization record
- Proof of Municipality of Anchorage residency – utility bill (gas, electric, garbage, water), rental agreement, or mortgage statement.
