Nurse Directory

New-To-District Students
Required to provide a health history form and immunization records to the school nurse.

Returning Students
Submit new immunization records and new health concerns to the school nurse.

Parents/guardians of students with health concerns should reach out to their school nurse to develop a plan for nursing services. Working together with you and your doctor, our nurses provide frequent services for:

• Daily medication needs
• Allergies/Anaphylaxis
• Asthma
• Diabetes
• Seizures
• Tube feedings

Forms

Throughout the school year, our nurses will provide vision and hearing screenings according to AS 14.30.127(a). However, if at any time a teacher or parent has a concern, they can reach out to the school nurse.
