New-to-District Enrollment

• Current IEP and Program Eligibility Reports
• Guardianship paperwork if the enrolling adult is not listed on the birth certificate
• Custody agreements or court documents affecting the student
• Tribal/Village Enrollment documentation or Certificate of Indian Blood for students enrolling in the Indigenous Education Program

Birth Certificate
Order a certified copy through the State of Alaska Vital Records.

Immunization Records

• Access your student's Alaska immunization records online though Docket.
• Download exemption forms through the ASD Immunizations webpage.

Proof of Residency
Access account portals for:

• Chugach Electric Association
• Matanuska Electric Association
• ENSTAR Natural Gas
• Anchorage Water & Wastewater Utility
• Alaska Waste
• Aurora Military Housing

Important: Phone and internet bills that do not display a service address cannot be accepted.

Unofficial Transcripts
Resources for obtaining records from:

• Alaska School Districts Information
• Alaska Public Schools Database
• DOWEA Schools

If the parent or guardian's name does not appear on the residency document, ASD may accept:

• A utility bill, lease agreement, or real estate contract from the resident, and
• A signed and dated statement from the resident confirming that the student and parent/guardian live at that address.

Additional documentation may be requested.

ASD's Child in Transition (CIT) Program provides enrollment assistance for students and families who meet the McKinney-Vento definition of homelessness.

A family may qualify if they lack a fixed, regular, and adequate nighttime residence.
