No medication may be administered without proper written consent/permission by the parent/guardian and /or the physician. The school nurse will provide you with the proper form for the type of medication. All medications must be brought to the nurse in their original container, labeled with the student’s name and accompanied by the proper form. Instructions for administering medication will include identification, quantity, purpose and possible side effects. All medications must be delivered to the school nurse by the parent/guardian unless approved by the nurse and school administration. Children are not allowed to have medications in their possession, except under special circumstances approved by principal and/or nurse. Please note that cough drops are considered a medication and must be brought to the nurse’s office to be administered to a student.
