- Medical Exemption (A signed physician's statement (on the State Medical Exemption form) is required if there are medical reasons a child cannot be vaccinated. Only a physician (MD/DO), physician’s assistant or advanced nurse practitioner licensed in Alaska is authorized to exempt children from immunizations.
- Religious Exemption (Notarization and the annual renewal are required. Form valid from July 1 through June 30) La Nore and Margie are both Notaries in the Frontier's Anchorage office. Either one would be more than happy to notarize your Religious Exemption. This is available only for Frontier Students.
- Child in Transition/CIT Waiver (30-day immunization waiver for the student in the Child in Transition program)
- Active Duty Military Waiver (Active Duty Military only 30-day immunization waiver- New to Alaska Arrivals)
