The AMCS Program Withdrawal Form is for students wishing to transfer out of the program. The AMCS Team must ensure the student has spoken to their advisor and counselor. Students need to complete the following:
- Withdrawal from college courses
- Return all college textbooks
- Ensure student financial account has no balance
The student must complete the form and send it to their parent's email address for signature.
