• The AMCS Program Withdrawal Form is for students wishing to transfer out of the program.  The AMCS Team needs to ensure the student has spoken to their advisor and counselor.  Students need to complete the following:

     

    • Withdrawal from college courses
    • Return all college textbooks
    • Ensure student financial account has no balance

     

    The student needs to complete the form and send it to their parent's email address for signature.


    You will need to enter your UAA Advisor's information prior to entering the form:
