Students must be up-to-date with all state-required immunizations.
4 AAC 06.055 requires children enrolled in Alaska schools to be immunized against:
- Diphtheria, Tetanus, Pertussis (DTap/DT/Td/Tdap)
- Polio
- Measles, Mumps, Rubella (MMR)
- Hepatitis A
- Hepatitis B
- Varicella through 6th grade
- Hib (Haemophilus influenzae type b) – under age 5 years only
The number of doses and type of vaccine required depends on your child’s age, grade and prior vaccination history. Check with your healthcare provider for details.

Exemptions
Complete and submit the following forms if you are seeking an exemption for your child.
- Medical Exemption - A physician's statement on the State Medical Exemption form is required if there are medical reasons a child cannot be vaccinated.
- Military - Federal law allows for a 30-day provision.
- Child in Transition - Federal law allows for a 30-day provision.
- Religious Exemption - form must be notarized and submitted each school year.
